  • What It Will Do. Upper lid surgery will make your eyes look brighter, larger, and more rested by removing loose skin, stretched muscle, and ex­cess fat. It should give you visible eyelid above the lashes, so that eyeshadow will show. Your eyes will look more youthful, because loosened skin and muscle are associated in people’s minds with old age.

  • What It Won’t Do. Upper lid surgery will not erase, remove, or change the wrinkles and crow’s feet at the corners of your eyes. Also, it won’t lift your brow. It won’t make your eyes symmetrical, and with the excess skin gone, you may notice asymmetry in your upper lids that you didn’t see before.

What is eyelid surgery?

Eyelid surgery is a surgical operation for removing “bags” under the eyes, restoring the line of eyelids or tightening eyelids so that to help you look younger. However eyelid surgery won’t  help you to remove wrinkles around the eyes or dark circles.

What happens before the operation?

If you consider having eyelid surgery you need to find a good surgeon who has experience in cosmetic surgery. At your first appointment you need to explain what your expectations are and get as much information about the operation and its results as possible. You need to be fully informed before taking the final decision. The surgeon will also study your medical history to make sure you don’t have any reasons not to have an eyelid operation. After that you will sign the consent to have guarantee that you are aware of possible risks and complications of eyelid surgery. Pictures of your eyelids could also be taken before and after the procedure to see the result. You might be required to have a test for glaucoma to make sure you don’t have any troubles with tear production.

What kind of anaesthetic is used during eyelid surgery?

Generally local anaesthetic is used, in some cases it is combined with a sedative. In rare cases general anaesthetic is required. Anyway no type of anaesthetic would be given to you without discussing it with you first.

What happens during the eyelid plastic surgery?

There are two different techniques of eyelid surgery for upper eyelid and lower eyelid.  In both of them the cut is made either with scalpel of with a laser. It is considered that there is less bleeding and bruising when using a laser. During the upper eyelid surgery a cut is made along the crease of the eyelid, then the excess skin is removed, if there is some unwanted fat then it is also removed, muscles are tightened to make a nice shape and the stitches are applied. Usually the scar is hidden in the natural fold of the upper eyelid so it is unseen. For lower eyelid there are two methods of cosmetic surgery:

First method is to make a direct cut below the lashes, then to remove the excess skin and fat and to apply the stiches.

The second method is used when you have no excess skin and just need to remove some fat from lower eyelid. The benefit of this method is that it leaves no visible scars as the incision is done at the inner part of the eyelid.

There is also a new procedure called S.O.O.F. (Sub Orbicularis Oculi Fat), during this procedure fat is not removed but lifted behind the muscles underneath the eye. The benefit of this technique is that there is no risk of sunken eyes appearance afterwards.

How long is the eyelid plastic operation?

Eyelid surgery lasts two or two and a half hours for both upper and lower eyelid on each eye. Some patients go home afterwards and some prefer to stay in a clinic overnight.

What to Do About Pain

Your upper lids may ache, starting an hour or so after surgery. Bending over may make them throb. Severe pain or intense throbbing is very rare, and you should call your doctor to alert him if this occurs. Possible reasons for severe pain include a bandage that is too snug a stitch that has come out, or unex­pected bleeding.

The Healing Process

Your upper lids will be swollen and sore for about 5 days. Bruis­ing varies greatly, from minor (gone in 7 days) in one person to major (traces lasting up to 1 month) in another. Your eyes will be sensitive to light, wind, and dust for 2 to 4 weeks. Your upper lids may be irritated and teary for about 14 days. Because your eyes will be irritated easily, and to avoid pulling on the surgical incision, you will not be able to wear contact lenses for about 10 to 14 days. Usually, you see your surgeon again on days 3 and 6 and then 2 weeks and 4 weeks after the operation.

Bandages

Bandaging is usually tape or a little strip of gauze over your stitches. Some surgeons will bandage your upper face with an Ace bandage and keep you in bed for the first 24 hours after surgery to rest your eyes. However, the trend is away from this, because it seems to be unnecessary.

How long is the recovery period after eyelid surgery?

It takes about 10 days maximum for swelling and bruising to subside so most patients are able to go out after this period of time.

What are the complications of eyelid cosmetic surgery?

During the first days after eyelid surgery it is normal to feel some pain and have swelling at the treated area. Swelling will subside in about 10 days. In some cases there might be bruising. There are also some possible temporary side effects which include dry eyes, blurred vision, difficulties with closing the eyes, excess tear production. In rare cases there might be infection, nerve damage, reaction to anaesthetic.

What should you do after eyelid surgery?

It is very important to follow your surgeon’s recommendations if you want to recover as quickly as possible. These recommendations may include taking painkillers to reduce pain after the plastic surgery in a first few days, applying cold compress to the treated area to reduce swelling, avoid physical exercising until your doctor would allow you to resume it, bathing the area around the eyes. And if you feel sudden increase in pain or any signs of infection you should contact your doctor immediately.

Back to Work

Most people feel and look ready to work about 1 week after surgery. However, if you’re extensively bruised or more than normally self-conscious about the surgery, you may decide to stay out of the public view for a few days longer—as about one- quarter of patients do. Even after 2 weeks, you may find that, although you are much better than you were, your upper lids still feel stiff and sore.

Back to Parties

You can usually count on looking and feeling right for a party 2 weeks after upper lid surgery. If you aren’t self-conscious and don’t mind people noticing and commenting on your surgery, you might feel like a party as early as 1 week but the late night (and alcohol) may leave you tired and may make your eyes swell the next day. If you want to be absolutely safe, postpone parties for 3 weeks.

Back to Sex

Your sutures are removed within a few days, so even a minor blow to your face could pull the fragile eyelid skin apart. This would give you a wide or thick scar. Postpone sex for at least 10 days.

Back to Sports

You can begin stretching exercises at the end of a week, but don’t do bending or lifting. It will make your eyes ache and swell. After 2 weeks you can resume swimming aerobics, and other sports—but not ball sports. (An exception is tennis. You can play a baseline game, but you should not play at net yet.) After 3 weeks you can resume all sports, but start slowly and build up. You will not have your presurgery endurance at first.

Who shouldn’t have eyelid cosmetic surgery?

You shouldn’t have eyelid surgery if you are not sure about the procedure, you should have realistic expectations. If you have some eye problems such as glaucoma or problems with tear production then you might not be a suitable candidate for eyelid cosmetic surgery. If you are prone to keloid scars then you should be ready that the scars would be visible and permanent, your surgeon would warn you about this.
